Comparison of total hip arthroplasty and bipolar hemiarthroplasty for Ficat stage III osteonecrosis of the femoral head

Background: Patients with Ficat stage III osteonecrosis of the femoral head (ONFH) usually require hip arthroplasty. Purposes: This study retrospectively analyzed the data of patients who underwent hip arthroplasty at our hospital. Methods: Patients who received bipolar hemiarthroplasty (BHA, n = 20, 22 hips) were compared with those who received total hip arthroplasty (THA, n = 19, 25 hips) in terms of survivorship and final function and pain scores. Results: The average follow-up period was 10.9 years. The BHA group had a higher revision rate than the THA group (22.7% vs. 0.0%, p = 0.017). The BHA group who did not undergo revision surgery had higher groin pain scores, without significant differences. However, pain scores evaluated based on Harris hip scores were significantly lower for the BHA group than for the THA group (34.9 ± 12.2 vs. 43.2 ± 1.6, p = 0.024), although their functional scores were similar (37.3 ± 8.6 vs. 39.5 ± 7.3, p = 0.439). Conclusion: In patients with ONFH, the survivorship and groin pain of the patients who underwent BHA were inferior to those who underwent THA. In those patients who did not undergo revision surgery, the patients who underwent BHA or THA had similar functional status, but the pain scores were higher in the BHA group. THA is recommended for patients with ONFH who require arthroplasty.
