Multifocal Cholangiocarcinoma Treated with Pancreatoduodenectomy and Left Lobectomy of the Liver-A Case Report

摘要

膽管癌在治療上對外科醫生而言現今仍然是一項挑戰，因為這種疾病並沒有一個共通性。不同的組織學，腫瘤位置和治療方法都可能影響到最終的結果。多發性膽管癌（肝內膽管癌併肝外膽管癌）是非常少見的或者其實是被低估了。罹患這種疾病要能夠成功地治療並長期存活，是非常困難達成的。我們報告一位47歲婦女成功地以Whipple（惠伯爾）氏手術結合左葉肝臟切除術來治療其所罹患的多發性膽管癌（肝內膽管癌併肝外膽管癌）。到目前為止此病患已存活65個月而且沒有復發的情形。

Cholangiocarcinomas still pose a therapeutic challenge for the surgeon, as there is no common entity for these diseases. Different histologies, tumor locations, and treatment techniques all affect the ultimate outcome. Multifocal (intrahepatic and extrahepatic) cholangiocarcinomas are rarely encountered and are often underestimated. Long-term survival is rarely achieved. We report on a 47-year-old woman with concomitant intra- and extrahepatic cholangiocarcinomas successfully treated by a combined Whipple's operation and left hepatectomy. Long term disease free survival was achieved without evidence of recurrence for 65 months to date.
