影響醫師實施臨床指引意願之因素探討－針對臨床指引本身之特性

摘要

本研究以結構性問卷郵寄抽樣訪問全國醫師，於92年10-11月之間以郵寄問卷方式進行，並於三週後再郵寄一次信函催收。回收率為17.8%。
臨床治療指引之實施在現階段及未來的醫療中必然成為一個趨勢。然而，醫療團隊的主導者醫師的意順勢必加以重視，在設定、實施所謂的臨床指引時務必考量醫師的想法。本研究顯示，臨床指引本身之特性中影響醫師實施臨床指引者為：「符合學理及證據醫學」、「由專科醫學會制定」、「執行時可以依情況修正」、「由獨立公正自主之機構監測」、「制定為全國性之指引」、「運用時必須在法律事件上對醫師有保護作用」及「可由期刊、刊物取得之可近性」都是在設定這個指引時必須加以注意的因素以提高醫師使用之意願。
綜合上述，本研究可了解醫師族群對臨床指引之期盼。政府應成立國家級之獨立機構（類似美國National Guideline Clearing House），結合實證醫學之運用（類似國外Cochrane Library），研發、制訂符合證據醫學、具有彈性、容易取得之全國性臨床指引並包括對於法律層面之探討，以推廣及監利臨床指引之實行與運用。

The clinical practice guidelines that have the characteristics such as 「match the evidence-based medicine」、「made by association of specialist」、「flexible in use」、「proctective role in legal events」、「national level」、「available on journals」 and 「monitoring by an independent organization」 will be favored by physicians in Taiwan.
