皰疹狀皮膚炎－發生於5歲男孩併發線狀IgA, IgG, IgM, C3之沈著－
Dermatitis Herpetiformis in a 5 Year-old Boy with Linear IgA, IgG, IgM and C3 Deposits
摘要
病人是5歲男孩，因全身劇癢，有紅斑及水皰而住院。皮膚組織病理檢查呈現表皮下水皰及真皮乳頭部有中性球爲主之微細膿瘍(microabscess)形成。由水皰週圍紅斑部之皮膚施行直接免疫熒光檢查(direct immunofluorescent test)，證實有IgA, IgG, IgM及C3之線狀(linear)沈著於表皮真皮境界部，由皮疹，組織病理學及直接免疫熒光檢查之變化，診斷爲皰疹狀皮膚炎(dermatitis herpetiformis)之病例。其臨床與組織病理學的變化與成人發生之皰疹狀皮膚炎相同，但大多數發生於小兒之病例其直接免疫熒光檢查爲線狀IgA，而成人者則爲顆粒狀IgA的沈著。本病例之直接免疫熒光檢查則同時出現線狀IgA, IgG, IgM及C3爲其特殊之變化，此種反應型式在文獻上尚未見報告。

A 5-Year-old boy was admitted with the chief complaints of generalized intense itching sensation, erythema, and vesicle formation for three weeks. The physical examination revealed symmetrical distribution of grouped vesicles, urticarial papules and plaques. The dermatopathological examination from an erythematous area showed subepidermal vesicles with neutrophilic microabscess at the dermal papillar areas. Direct immunofluorescent test at the perivesicle area revealed linear pattern of IgA, IgG, IgM and C3 deposits at dermoepidermal junction.
