三維電腦斷層影像運用於測顱術之技術開發

Three Dimensional Computed Tomography Utilized in Cephalometry-A Technical Investigation

摘要


現行牙科X光測顱術多半採用側面平面X光片作為顱顏面形態分析的依據,但此種分析方法受限於影像之來源為二維之平面影像,對於複雜的立體解剖構造的解析有所困難。本研究提出創新之方法,以三維電腦斷層影像為基礎建立立體頭顱影像,並設計親善與簡單的使用者介面,顯示整體的空間概念供使用者明確的辨識及定位測顱特徵點,並依Downs分析法進行測顱術分析。研究中以10位觀測者在二個不時間點,依上述方法進行測顱,評估此方法的準確性和再現性,作為測定系統可行性之初步研究。結果顯示上述方式為一可靠的測顱方法,值得未來再進一步深入研究。

Current cephalometry in dentistry relies on plain lateral films to evaluate craniofacial structure of patients. This concept is based on 2D images which are difficult to apply in complicated craniofacial structures. The purpose of this study is to develop a 3D cephalometric analysis by reconstructing the 3D-CT skull images, which provides landmark identification and Downs analysis in the 3D software. This system also has the feature of multiple view windows, auto cephalometric measurement and counting, and simplified user's interface. A pilot study, using the landmark identification and cephalometric analysis which was processed by ten observers at two different times, was conducted to evaluate the accuracy and reliability of this system. The result supports this approach to be a reliable method in cephalometric analysis and worthy to research in the future.
