評估連續收取SPECT造影方式之可行性

Evaluating the Feasibility of SPECT Imaging with Continuous Acquisition Mode

摘要


動機:(上標 99m)Tc-DMSA腎臟皮質檢查是診斷急性腎盂腎炎的常規檢查項目,其非侵入性檢查的特性應用於嬰幼兒又可兼顧安全照護及易於安撫的優點。但(上標 99m)Tc-DMSA腎臟皮質攝影檢查需耗時約三十分鐘,檢查進行時受檢者需保持身體姿勢靜止不動,於嬰幼兒甚有困難。故常要求於入睡狀態進行檢查,然而部分SPECT機型在旋轉過程中機械聲響過大,尤其是每次旋轉於攝影定位時的“喀啦”聲恐將淺眠的孩童擾醒,進而使檢查失敗或影像位移。爲了解決此一狀況,考慮以連續攝影方式進行影像光子的收集,以消除SPECT攝影時之最大噪音,使檢查順利進行。但若以連續方式收集影像,影像品質勢必受到影響,本研究即針對改變影像收集方式所造成之影像解析度進行評估,並藉由分析結果歸納出不降低影像解析度時應採用之攝影條件。材料與方法:本研究對單光子斷層影像的評估,利用Flanged Jaszczak ECT Phantom於西門子E.CAM進行連續及非連續性收集影像條件測試,分別以每張影像30,37,與45秒,以及每一偵測頭收取32與48張影像進行不同攝影條件之影像擷取。並利用直線剖面圖呈現假體影像中冷區的影像解析度,同時找出最佳連續攝影條件。結果:連續性方式攝影以不同時間與張數(30,32),(37,32),(30,48),(45,32),及(45,48)進行分析,其結果與非連續性方式攝影時,對影像冷區表現有顯著性差異之比例分別爲0,60%,40%及100%。結論:執行嬰幼兒單光子斷層攝影時,若因臨床狀況需環境降噪,使用連續性造影方式之最佳參數爲收取每張30秒與每個偵測頭32個轉面(view)進行單光子斷層攝影;不但對假體影像冷區影像品質沒有差異性(p=0.33488~0.92903),且能以較經濟的攝影時間完成。

Motivation: The (superscript 99m)Tc-DMSA renal cortical scan is a regular examination in diagnostic acute pyelonephritis. It is non-invasive, safe care, and soothing relief for babies. However, it needs thirty minutes for (superscript 99m)Tc-DMSA renal cortical scanning. Static mode is difficult to image babies and young children without motion during thirty minutes scanning. Conventionally, (superscript 99m)Tc-DMSA is often required to image and process scanning while baby or child is asleep. Nevertheless, SPECT images are noisy during the rotation. Especially, the SPECT makes louder noise during rotation of views and often awake the sleeping child. That leads to examination failure or image displacement. In this study, the main aim is applied continuous mode in SPECT scanning protocol to eliminate the motion artifact and to provide smoothing examination for child. However, the image quality will be affected certainly under continuous mode. For this reason, we analyze and find scanning protocols and parameters without decreasing image quality under continuous mode. Materials and methods: The imaging scanner is Siemens E.CAM. Flanged Jaszczak ECT phantom was applied to investigate scanning protocols and conditions between continuous mode (CM) and step and shoot mode (SSM). There were three different acquisition times including 30, 37, and 45 seconds per view. 32 and 48 views per head were designed to evaluate the quality of images. The optimal scanning conditions and analysis of image quality were investigated and compared with cold spots of phantom images. Result: There is zero, 60%, 40%, and 100% significant between CM and SSM while time of acquisition and views of head were (30, 32), (30, 48), (45, 32), and (45, 48) with respectively to seconds and views. Conclusion: In this study, the optimal scanning conditions were 30 seconds of view and 32 views of head. It not only reduces louder noise, but also keeps quality of image between SSM and CM. Meanwhile, CM of SPECT imaging is feasible to apply baby or young child.
