本文為描述一位嬰兒感染大腸桿菌併發腦膿瘍術後,由加護病房轉至兒童病房的護理經驗,護理期間為2018年12月20日至2019年1月16日,藉由臨床照護、身體評估及會談進行評估,確立健康問題有現存性感染、潛在危險性傷害、腹瀉及照顧者角色緊張等。運用發展性支持照護,協助嬰兒緩和壓力,促進及統合其神經系統,避免合併症發生;因長時間接受抗生素治療導致腹瀉,協助緩解腹瀉並避免因糞便浸潤導致之皮膚破損,指導照顧者對長期腹瀉的照護;運用傾聽、同理及鼓勵主要照顧者表達其內心感受,提供疾病相關資訊及照護技能,利用家庭支持系統,緩解照顧者對於嬰兒住院之壓力感受。希望藉由此次嬰兒術後需長期住院及其家庭的身心護理經驗,分享同仁運用護理專業技能及關懷,提升更完整的照護品質。
This article discusses the experience of providing care for an infant with brain abscess complicated by postsurgical Escherichia coli infection who was transferred from the intensive care unit to the pediatric ward. The nursing period was from December 20, 2018, to January 16, 2019. The patient was assessed during clinical care through body evaluations to monitor the existing infection, risk of injury, diarrhea status, and potential caregiver role strain. Developmentally supportive care was used to relieve the infant's stress, boost and unify the infant's nervous system, and avoid complications. Long-term antibiotic treatment was required to cure the brain abscess, but antibiotics would cause diarrhea. This study aimed to relieve the patient's skin impairment due to the infiltration of fecal matter, provide caregivers with guidance on how to take care of the infant using a listening with empathy strategy, and encourage caregivers to earnestly express their feelings. We provided caregivers with knowledge of nursing skills applicable to the disease and used family support systems to relieve caregivers' stress. We hope to improve the quality of nursing offered in the field by sharing our experience of applying specific nursing skills in this particular case.
