We present a case of paraganglioma of the filum terminale. The patient was a 45-year-old man, who suffered from low back pain while coughing for one month. MRI revealed a 2cm diameter intradural tumor at the level of L3. The patient received a lumbar laminectomy with total tumor removal. He recovered uneventfully after the operation. Pathological examination showed a picture of paraganglioma. A paraganglioma of the filum terminale is extremely rare. However, we should keep in mind this possibility when a highly vascular lesion is seen in the region.
