本專案旨在提升口腔癌病人術後口腔清潔指導完整性,於2020年1月調查口腔癌病人術後未執行口腔清潔65%、口腔清潔指導完整率僅62.2%,且調查護理人員執行口腔癌病人術後口腔清潔之認知,結果顯示正確率為55.9%,調查分析原因為:1.缺乏在職教育訓練2.缺乏照護標準及監測3.缺乏衛教單張。專案小組介入措施包含:制定口腔癌病人術後口腔清潔照護標準及監測、舉辦教育訓練、製作衛教單張及提示小卡。專案實施後口腔癌病人術後口腔清潔指導完整率提升至94.3%、護理人員執行口腔清潔認知提升至98.8%,達成專案目的,使病人得到完整之照護,並降低感染發生。
This project aims to improve the integrity of oral care guidance for patients with oral cancer and reduce the likelihood of infection. In January 2020, 65% of patients with oral cancer did not do oral care after their operation. The completion rate of oral care which is guided by nurses was only 62.2%. In addition, our investigation revealed that nurses perceived the correct rate was 55.9% of oral care for post-operative cancer patients. Our analysis suggested the reasons might be: 1. lack of on-the-job training, 2. a lack of standard operation procedure and monitoring system, and 3. no related health education papers. Thus, our intervention including establishing standard operation procedure and monitoring system of oral care for post-operative patients, holding on-the-job training and making health education papers. After adopting strategies mentioned above, the completion rate of nurse guided oral care increased to 94.3%, and nurses' perception of oral care for post-operative cancer patients rises to 98.8 %. The results of this project enable a much greater number of patients to receive complete care and help in reducing the infection rate.
