A case of congenital esophageal atresia with tracheo-esophageal fistula in a full term girl baby is presented. It was diagnosed when the patient was still alive by fluoroscopic examination with a barium-filled Nelaton catheter which failed to enter the stomach and kinked at the level of the second thoracic vertebra. The air in the stomach and intestine assured us the presence of the tracheo-esophageal fistula. After the diagnosis was established she was sent to the operating room for emergency operation, but she died of suffocation on the way of transportation to the operation room. After the death of the patient fluoroscopic examination was undertaken. Barium was introduced with a nasal tube and the blind upper esophageal segmeat was demonstrated. By injection of barium into the trachea, the lower segment of the esophagus which joined the trachea at just above its bifurcation was also demonstrated. The above mentioned findings were confirmed by autopsy.
