先天性結核症之一病例報告

摘要

筆者報告一個先天性結核症，具有多發原發性複合竈(Multiple Primary Complexes)的病例。其傳染路徑可能：
1.純胎盤感染：細菌經由胎盤入胎兒體內，一部分停留在肝臟，而另一部分通過肝臟而進入肺部，因此在肝臟及肺臟，都有原發性複合竈存在。
2.胎盤感染與羊水吸入的肺臟感染，同時發生。
筆者並參閱文獻，討論先天性結核症的臨床症狀，診斷及預後等。並對臺灣地區結核症的預防計劃加以檢討。

A case of congenital tuberculosis with primary complexes in the liver and lungs is reported. The possible ways of transmission were 1. Pure transplacental infection: part of the bacilli may get lo lged in the liver via the placenta and part of then passed through the liver and settled in the lung si nultaneously. 2. Transplacental transmission and aspiration of infected amniotic fluid happened at the same time. The clinical course and diagnosis of congenital tuberculosis are discussed. The tuberculosis control program in Taiwan is also discussed.
