摘要
在小孩急性腦症中，Reye氏症候羣（急性腦症加上內臟脂肪變性）乃是常見之一，在臺灣亦不少見。本報告包括過去兩年來發現的14個病例，其中8個內臟在病理上有脂肪變性。全部病人都有前驅症狀，以輕微的病毒體感染現象最為常見。臨床過程與症狀都很類似。肝機能檢查方面，除一個病例外，轉氨基酶都昇高，但血清膽紅素卻正常或僅輕度的增加。活存病例追蹤中，顯示肝機能很快恢復正常。10個病例死亡，一個活存的病例有嚴重的腦傷害。兩個病例接受腹膜透析，一例活存，另一例死亡。此病之原因尚不明，但病毒體被認為是最有可能的原因。

Reye's syndrome (acute encephalopathy with fatty degeneration of viscera) is not uncommon in Taiwan. Fourteen patients with the syndrome had been studied over a 2-year period, eight of them had fatty degeneration of viscera which were proved either by necropsy or biopsy. The ages of them ranged from 3 months to 12 years. Eleven were male and 3 female. Clinical features of the disease were found to be similar. All had a mild prodromal illness, mostly consisting of a non-specific febrile illness with cough, rhinorrhea or mild gastrointestinal disturbance. It was followed by vomiting and then development of an encephalopathy without a concomitant encephalitis or meningitis. Fever, respiratory disturbance, consciousness change and hepatomegaly were the main clinical features. Selected liver function tests were checked in twelve patients. GOT was elevated in all except one, who showed a typical fatty metamorphosis of liver. Serum bilirubin remained normal or slightly elevated. In the survivors, there was a very rapid return of the liver function to normal. There were 10 fetal cases. The overall mortality rate was 71%. Of the 4 survival patients, one had severe brain damage. Two patients were treated with peritoneal dialysis, of whom one survived and one died.
