摘要

在過去六年間，本院經驗了35個新生兒破傷風病例。有16例痊癒另19例死亡，死亡率高達54.3%。住院年齡愈大或潛伏期愈長的其死亡率愈低。住院中體溫不穩的死亡率亦高。破傷風免疫球蛋白的用量多少與死亡率沒有什麽特別的關連。Valium加上其他鎮靜劑可減少死亡率。護理是減低死亡率的最主要治療方法。

During the past six years from January 1971 to December 1976, we experienced 35 cases of neonatal tetanus at the Mackay Memorial Hospital. Males and females were affected equally. The majority of them were delivered under unsterile condition. The age on admission ranged from 3 to 16 days, and the incubation period from 2 to 14 days. The case fatality was 54.3%. Frequent apnea attack was the main cause of death. All but one expired within one week after the admission. The average hospital stay of the 16 survivors was 26.4 days.
Trismus, refusal of feeding, twitching, opisthotonus and fever were the common symptons and signs. There was no significant difference between the mortality rate and dosage of tetanus immunoglobulin (TIG). Valium combined with one or two other sedatives reduced the case mortality significantly.
Many factors including body temperature, incubation period, administration of TIG and sedative can influence the fatality rate. Among which the nursing care is the most important one.
