摘要


馬偕紀念醫院在民國67年6月及7月,連續發現2例小兒鉛中毒腦症。一個爲15個月大,一個爲9個月大。前者家中經營陶器工廠,患者常以手沾食上釉之鉛粉;後者因左右隣舍皆爲噴漆工廠,長期曝露在鉛源環境中而得病。兩者皆有很明顯的腦壓增高及抽搐現象;周邊血液抹片,在紅血球內均有明顯的嗜鹼性斑;長骨照射也都呈現明顯的鉛緣。前者血鉛定量爲47μg%,後者血鉛定量爲180μg%。前者來不及治療就已死亡;後者經EDTA及BLA治療後,症狀改善出院。

Two cases of lead encephalopathy were diagnosed at the Mackay memorial Hospital, in June and July, 1978, respectively. The first patient was a 15-month-old baby girl who was admitted on account of vomiting for 2 days. The signs of encephalopathy occurred during her second hospital day. The peripheral blood smear showed anemia and basophilic stippling of red cells. Roentgenoscopicall, dense bands at the metaphyses of the long bones were also noted. A review of the history revealed that the child had been living in a family type of pottery glazing work shop, and had a habbit of pica. The blood lead level was 47μg%. She expired before starting treatment with the chelating agents. The second patient was a 9-month-old little boy who was hospitalized with intractable seizure attacks and consciousness disturbance. The peripheral blood smear and the long bone X-ray films showed the characteristic pictures of lead poisoning. The blood lead level was 180μg%. The urine lead level was 93μg%. He might have had excessive lead exposures and became poisoned as he was living in an area where there were many spray painting factories. He improved after EDTA and BAL administration.
