摘要

本報告係研究Beclomethasone dipropionate鼻噴霧劑對過敏性鼻炎之治療效果。我們選擇20位氣喘症狀已減輕但鼻炎症狀仍然嚴重之過敏性氣喘病兒，每天每個鼻孔噴入150μg~200μg(即3-4puffs)，速續試用4星期。結果發現1.本製劑對所有病兒均有效，且在使用3天後，症狀即開始有明顯之改善，2.血中及鼻黏液之嗜伊紅性白血球在使用3天後均降低，3.鼻黏膜腫脹明顯減少，4.無任何重大副作用。是故beclomethasone dipropionate短期使用可迅速有效的控制過敏性鼻炎症狀，且對身體沒有傷害。

The efficacy of intranasal beclomethasone dipropionate aerosol in the treatment of allergic rhinitis was studied in 20 well-controlled asthmatic children with severe allergic rhinitis. All 20 patients were skin test-positive to at least one of the 16 allergens used in our allergy clinic; nineteen cases had positive family history of allergy; 54% (7/13) had elevated serum IgE and two thirds (10/15) had increased total eosinophil count (＞400 cells/mm^3). Total eosinophil count, nasal eosinophilia and the degree of hypertrophy of nasal turbinates were checked before and after completion of the study. Subjective symptoms such as nasal obstruction, itching. Sneezing and rhinorrhea, were recorded daily by patients the mselves or their parents. Beclomethasone dipropionate in the aerosol form (Aldecin nasal spray, Schering Corporation, U.S.A) was introduced into each nostril one puff (50μg) at one time, t.i.d. or q.i.d. for four weeks. The results showed: 1) all patients responded to this therapy very well and the symptoms improved about three days after starting of the treatment, 2) the average total eosinophil count significantly decreased after the treatment (P＜0.05), 3) before the treatment, nasal eosinophilia was present in 12 out of the 14 patients examined but only four patients still had nasal eosinopilia was present in 12 out of the 14 patients examined but only four patients still had nasal eosinopilia after the treatment, 4) the hypertrophy of nasal turbinates diminished markedly after the treatment and 5) no serious side effect was found. In conclusion, shout-term (four weeks) local aerosol administration of beclomethasone dipropionate was effective in the treatment of severe allergic rhinitis and no serious side effects were found.
