海綿竇內內頸動脈瘤引起眼瞼下垂眼肌麻痺及硬腦膜下血腫之一病例報告
Internal Carotid Aneurysm Causing Ophthalmoplegia and Ptosis: A Case Report
摘要
本文報告一例10歲男童，呈現眼瞼下垂、眼肌麻痺及三叉神經感覺異常等臨床症狀。經電腦斷層攝影及腦血管攝影檢查發現爲內頸動脈瘤。病人於接受總頸動脈結紮3個月後，突發蜘蛛膜下腔出血及硬腦膜下血腫。乃追加施行動脈瘤遠心端結紮術，術後情況良好。動脈瘤於小孩時期並不常見，內頸動脈瘤在臨床上有其特殊徵象故提出報告，以供參考。

Aneurysm arising from the internal carotid artery behaves differently from aneurysms arising elsewhere in the skull because it traverses the cavernous sinus. We are reporting a 10-year-old boy with an internal carotid aneurysm. He had sudden onset of headache, vomiting and ptosis of the right eye. Progressive ophthalmoplegia and hyperesthesia of the face developed subsequently. An intracavernous aneurysm locating in the right internal carotid artery was delineated by CTI scan and carotid angiography, One month after right common carotid artery ligation a subarachnoid hemorrhage developed. So the distal end of the aneurysm was clipped. No new neurologic defect occurred, and the ptosis and opthalmoplegia recovered gradually.
