早產兒袋鼠式護理對親子關係的影響之初探性研究

摘要

早產兒的誕生常會帶給父母許多的壓力與焦應，甚至進而阻礙父母與早產兒間親子關係的建立；文獻呈現早產兒袋鼠式護理，不僅可穩定早產兒的生理發展，更可有效地促進父母親與早產兒親子關係的建立，是一項兼具醫療與人性的照護方式。然而在台灣推行早產兒袋鼠式護理，對於父母與早產兒親子關係的建立是否亦具相同成效？本研究針對早產兒袋鼠式護理對增進父母親與早產兒親子關係建立之影響進行初步之探討。本研究以開放式自填問卷收集資料，以了解早產兒父母親參與袋鼠式護理之經驗及感受；所得16份資料採內容分析法，歸納、分析而呈現父母對袋鼠式護理的認知，自覺此方式對早產兒時影響，及執行袋鼠式護理對父母的影響等三方面，進一步探討袋鼠式護理對親子關係的影響。結果發現：袋鼠式護理可以降低早產兒父母親的焦慮，增加照顧早產兒的自信，並有效地促進父母親與早產兒親子關係的建立，故建議臨床推廣此有利於早產兒家庭的照護方式。

The coming of preterm infants usually brings their parents a variety of mental distress and anxiety or even worse hinders the establishment of the parent-infant relationship. Several studies indicated that ”kangaroo care” (KC) for preterm infants contributes not only to stabilizing the infants' physical development, but also to establishing the parent-infant relationship. Therefore, KC is a type of care which meets both medical and human needs. However, it is unknown whether KC is performed as well in Taiwan. This study explores how KC influences the interrelationships between parents and their preterm infants. Sixteen parents with KC experience were the subjects for this research. After giving informed consent, the parents filled up a questionnaire with eight open-ended questions to express their understanding and experience of KC. All data were analyzed by the method of content analysis to present the parents' understanding and appreciation of KC. The results showed that KC could decrease parents' anxiety, increase parents' self-confidence in caring for their own preterm infants, and promote the relationship between parents and infants. In conclusion, providing KC for preterm infants and parents is beneficial.
