本研究是利用質性研究,對在護理之家中工作的護理人員,進行深度的訪談與參與式觀察。目的在於探討護理之 家中的護理人員,所認為的護理之家照護品質的項目與標準。個案分別收案於基隆、嘉義、與高雄三所護理之家;樣本包括十位護理人員。研究發現護理人員描述護理之家照護品質項目可分為六大類:環境、品質管理、基本人權、照顧提供者的態度、社會互動、以及需求滿足,在品質管理方面,護理人員認為護理之家應有品質管理監測的工作流程,工作人員應該有進修機會,以提升人員的專業素質;在基本人權方面,護理人員雖然認為肢體的約束與忽略住民的自註性功能是不可行的,但仍會依情況而為之。本研究之結果,除可提供從事長期照護的相關人員,進一步了解護理之家中護理人員對照護品質之意見;也有助於訂定護理之家照護品質之指標。
A qualitative research method was used. In-depth interviews with participant observation were used to collect data, to explore the indicators of quality of care for nursing homes from the perspective of nurses. The sample consisted of 10 nurses from three accredited nursing homes in Keelung,Chia-I, and Kaohsiung. The study found six groups of indicators of quality of care for nursing homes: environment, quality assurance, basic human rights, direct care attitude, social interaction, and needs satisfaction. In particular, in terms of quality assurance indicators, nurses thought that quality assurance, such as the supervision process, and on-the-job training for stauff members to enhance their abilities, were important indicators. They also mentioned negative indicators of basic human rights. However, nurses believed that physical restraint was necessary, but must depend on the conditions. The findings of this study may sensitize long-term care professionals to the needs of nursing home nurses. They may also be useful for defining nursing home quality of care indicators.
