張景岳傷寒典外感病學術思想之研究

摘要

張景岳為明朝溫補學派著名的醫家，其所著傷寒典的內容為廣義傷寒，凡一切外感的疾病均以傷寒為總名，包括正傷寒、春溫、暑熱、瘟疫等，與張仲景傷寒論不同。本文旨在探討景岳傷寒典外感病的學術思想及用藥特點，結果顯示，景岳治外感病特別重視勞力傷寒，也說是夾虛傷寒。夾虛傷寒的病因病機理論，其思想乃源自於景去所創立的陰陽與命門學說。在治療上，夾虛傷寒分兩大類四個證型，一類為陽虛傷寒與陰虛傷寒，另一類為陰盛陽衰和陰盛格陽之傷寒，此四個證型均以溫補氣血及元陰元陽為主；在解表方面，陽虛傷寒與陰虛傷寒以柴胡為主，陰盛陽衰之傷寒以麻黃為主，陰盛格陽之傷寒則禁用任何解表藥，而以附子、肉桂溫陽散寒。

Zhang Jing-Yue was a famous physician of warm-supplemetary school in Ming Dynasty. The content of ShangHan Dian written by Zhang's was a book broaded definition of shang han. The all disease caused by external contraction of the evil was common named shang han. It included truth shang han, spring warm disease, summer heat-heat, and scourge epidemic. It was different from ShangHan Lun wrote by Zhang Zhong-Jin. The research of purpose was in order to discus the thought and the character of Chinese herbs treated external contraction disease in Shang Han Dian by Zhang's. Results demonstrated that Zhang's treating external contraction disease was especially appreciated the fatigue of shang han, an another named body vacuity with shang han. The thought of etiology and pathogenesis theories of the body vacuity with shang han was originated from Zhang's Yin-Yang and Life Gate hypothesis. On treatment of body vacuity with shang han was divided two type and four identify patterns. One type was yang-vacuity and yin-vacuity shang han patterns. Another type was shang han of yin-exuberance and yang-debility pattern as well as yin-exuberance and repelled yang pattern. The four identify patterns treatment was all used the method of warm-supplementary qi and blood as well as yuan-yin and yuan-yang. On the aspect of resolving the exterior, bupleurum was a main drug for yang-vacuity and yin-vacuity shang han pattern. As for the main drug of yin-exuberance and yang-debility pattern was ephedra. Any drug resolved the exterior in yin-exuberance and repelled yang pattern was contraindicated. Whereas, aconite and cinnamon bark were used to warm the yang and dissipate cold.
