飲食,是人們生活中每天所不能少的東西,它供應了體內所須的各種營養素(Nutrient),亦提供了足夠的能量(Energy),而使得個體得以保持著健康。然而,任何人都無法永遠處在於健康的狀態,也就是說,人們都有需要服藥的時候。過去在臨床上,醫師們對於所施用於病人的藥物,比較會考慮不同藥物彼此間可能有什麼作用,以及這些作用對於病人所可能產生的影響,也就是所謂的藥物間之相互作用(Drug-drug interaction)的問題^1。然而更重要的問題是在於藥物和營養素之間會產生怎樣的結果^2,卻是較少被注意的。如今,醫藥方面的知識已大大地進步了,因而發現了某些藥物可能會對我們體內所需之營養素有所影響^3,於是這方面的研究逐漸地受到重視,所以服入體內的種種藥物和體內所需之營養素二者之間的關係已逐漸明朗。本文擬就此作一概括性之介紹。
Drug-nutrient interaction is essential in clinical practice and also important in human medicine. This minireview will emphasize (i) the strikingly characteristic differences in absorption and metabolism of drugs and nutrients; and (ii) the elucidation and evaluation to the effects of drugs upon nutrient on the following categories: drug-food or drug-nutrient incompatibility; drug-induced changes in appetite; drug-induced malnutrition; drug pharmacokinetics in malnutrition; diet effects on drug disposition; pharmacologically active substances present in food-the interactions between tyramine-containing foods and MAO inhibitors; and the helpful or harmful effects of vitamin antagonists in management of patients. Thougn knowledge of these drug-nutrient interactions is becoming more apparent, we must continue to be aware of these interactions so that they may be dealt with intelligently.
