草藥引起抗膽鹼中毒:三病例報告

Herbally Induced Anticholinergic Intoxication: A Report of Three Cases

三個自行服用草藥的個案,於服用後短時間內產生急性腦器質症候群,症狀包括:意識不清、定向感喪失,自言自語、躁動不安、臉部潮紅、視幻覺、血壓升高、脈搏加快、噁心嘔吐、瞳孔放大及對光反射不良等,經檢查復診斷為急性抗膽鹼中毒。臨床上密切觀察及支特性療法,三個案均在24小時內完全恢復,未留下後遺症。經進一步探討分析後發現草藥中含有抗膽鹼成份,或可能相互作用,而引發急性抗膽鹼中毒;文中將進一步討論引起抗膽鹼中毒之機轉,診斷、藥物及治療。

The symptoms of anticholinergic intoxication are well established and present as delirium, disorientation, irritability, flushing, visual and auditory hallucinations, elevated blood pressure, tachycardia, nausea, vomiting, dilated pupils with sluggish light reflex, and other psychotic behaviors. Detailed history taking, physical and neurological examinations, and biochemical investigations are essential for making an accurate diagnosis of this syndrome. Three cases were examined, one male and two females. Their ages were 48, 31 and 24 year-old respectively. These subjects were diagnosed as having acute anticholinergic intoxication following emergency psychiatric consultation. Subjects had developed anticholinergic intoxication shortly after they had taken some herb drugs. One of the herb drugs use found to contain cholinergic, parasympathomimetic and diuretic effects, the other contained Datura metel L. which has been known to be a strong anticholinergic alkaloid, including scopolamine and atropine. The possible mechanisms of developing anticholinergic intoxication through ingestion of these herb drugs may include direct pharmacological effect of herb drugs and indirect interactions between herb drugs. Under close observation and supportive treatment they recovered within 24 hours without any sequelae. The pharmacological effects of a belladonna alkaloid in developing anticholinergic intoxication and its management are discussed.
