Nonparasitic True Splenic Cyst: A Case Report of Lymphangioma of the Spleen
真性脾臟囊腫：脾臟囊性淋巴管瘤病病例報告
摘要
脾臟囊腫並不是常見之疾病，比其他腹腔內器官囊腫要少見，但比脾臟腫瘤要多見。自1829年Andral首次於剖檢中發現第一例脾臟非寄生蟲性囊腫，至今約有600例報告。 脾臟囊腫以有無內襯表皮分為真性或偽性囊腫，寄生蟲性脾囊腫佔全部脾囊腫之三分之二。先天性真性脾囊腫却只佔百分之十，先天性脾囊腫中以血管瘤佔最多，以皮樣囊腫為最少，而脾臟囊性淋巴管瘤在兩者之間。 我們經驗有一位67歲榮民，因其他病情住院，而腹部超音波偶然發現貏臟有三個高回音之腫塊，經電腦斷層掃描、核酸掃描、血管攝影、皮下穿刺細胞學檢查均無法診斷出病情，最後經開刀後證實是脾臟囊性淋巴管瘤。特將此病例提出報告以供參考，並討論每項檢查之結果及回顧文獻以討論之。

Nonparasitic splanic cyst is not a common clinical and pathologic entity. In this study, a 67 year-old male patient was found to have 3 hyperechoic masses in the spleen incidentally during a routine examination of abdomen by sonography. A correct diagnosis of splanic cyst could not be made preoperatively using either ultrasound, Tc-99 phytate liver spleen scintigraphy, heat-damaged red blood cell splanic scintigraphy, computed tomography, or angiography and percutaneous fine needle aspiration cytology. Discussions of failures of each investigation and review of splanic cysts will be presented.
