後腹膜腔囊性淋巴管瘤：病例報告

摘要

後腹膜腔囊性淋巴管瘤是一罕見的良性腫瘤，通常在孩童時期即可發現，男女性在嶺生率上並無差異性，該腫瘤通常小於5公分且很少出現鈣化。在此我們提出一個發現在成年女性左側後腹膜腔的巨大囊性淋巴管瘤，其電腦斷層掃瞄及超音波影像顯示為一均勻低密度的囊性腫瘤，在腫瘤壁上有散在的點狀鈣化。由於影像並無特異性，加上其體積龐大(20X7X20cm)並緊壓附近的器官，故術前無法確定診斷。術後病理及腫瘤內含物的實驗室檢查發現該淋巴管瘤內有發炎及出血並含有高數值的澱粉酵素，由於病人以往並無任何疾病，故無法解釋該腫瘤內為何會存在高數值的澱粉酵素。而以往文獻未曾有淋巴瘤內含有澱粉酵素的相關報告，其間的關連性有待將來更進一步的研究。

Retroperitoneal cystic lymphangiomas are rare benign tumors which are usually presented in early life and rarely calcify. Here we have presented a huge retroperitoneal cystic lymphangioma with scattered calcifications in its wall in a 26- year-old female. The huge tumor compressed the stomach and pancreas and encased vessels of the spleen. It contained cloudy yellow fluid with high amylase value (446 U/L), inflammation and hemorrhage in this cystic tumor was also found. The past history is non-contributory including no trauma or pancreatitis. To the best of our knowledge, cystic lymphangioma with calcifications in the wall and high amylase in fluid content has not yet been reported.
