摘要


為了瞭解麻疹疫苗注射後的免疫力,在本院健兒門診以隨機取樣的方式收集了95位嬰幼兒,分別在麻疹疫苗注射前,注射後兩個月及其後每隔兩個月採血,以微量補體結合試驗法(microtiter complement fixation test)及間接免疫螢光抗體試驗法(indirect immuno-fluorescent antibody test)來測定血清中麻疹抗體的力價。結果發現母親遺留的抗體反應在滿13個月以後完全消失。1歲以後接種麻疹疫苗者其間接免疫螢光抗體試驗血清陽轉率及抗體的幾何平均力價皆顯著高於1歲以前接種者(X2 =6.0697,p<0.025;t=1.87,p<0.05)。且1歲以前第一次接種後抗體力價小於1:8者於滿15個月後再追加一劑,以微量補體結合試驗及間接免疫螢光抗體試驗來檢查仍然有53.8%及18.2%的接種者其抗體力價依舊小於1:8。再以1歲以前接種第一劑而滿15個月以後再追加者與1歲以後單劑接種者比較其抗體反應,結果發現1歲以後單劑接種者其微量補體結合試驗抗體反後優於1歲以前接種第一劑而滿15個月後再追加者(X2 =7.4319,p<0.01);間接免疫螢光抗體試驗抗體的幾何平均力價亦顯著高於滿15個月再追加者(t=2.13,p<0.05)。根據以上的結果結論是麻疹疫苗的接種在沒有爆發流行的情況下,宜延至12個月以後施行。

From December, 1985, till March, 1987, ninty-five healthy babies were randomly selected to evaluate the efficacy of the measles vaccine at the Pediatric Department of the Kaohsiung Medical College Hospital. Serum measles antibody titers were detected by the microtiter complement fixation test (CF) and the indirect immuno-flourescent antibody test (IFA). The results showed that maternal antibodies had completely disappeared by the age of 13 months. According to the child's age at the time of vaccination, the seroconversion rates of the CF and IFA tests were respectively 60.9% and 67.5% between 9 and 11 months of age, 80.9% and 87.2% between 12 and 14 months, 90.9% and 100% in the group over 15 months of age. The elder the baby when inoculated, the higher the positive seroconversion rate obtained. Babies inoculated beyond 12 months of age had higher geometric mean titer (IFA) than those inoculated below 12 months of age (p<0.05). 53.8% (CF) and 18.2% (IFA) of the children whose serum antibody titer after the first vaccination (<12 months of age) were less than 1:8, still had a measles antibody titer less than 1:8 after the booster vaccination at 15 months of age or older. Those who were inoculated with a single dose of the vaccine after 12 months of age had higher seroconversion rate (CF) and geometric mean titer (IFA) than those vaccinated before 12 months of age and revaccinated after 15 months of age. Thus, this study concludes that the appropriate time for measles vaccination should be deferred until 12 months of age when measles is not in epidemic.
