高雄市有慢性健康問題的老人利用社區醫療資源情形的探討

摘要

本研究為瞭解老人接受免費健康檢查技的就醫情形及障礙，而以高雄市八十年度十一區衛生所老人健檢稜有異常問題者為母群體，採酗額抽樣法，共抽得 661 名老人，利用結構式問春，由經過正式訓練的訪員，透過家庭訪視的方式收集資料，實訪 484 名，完成率73.2 %。所得資料分析結果如下：
一、經衛生所老人免費徒康檢查稜，獲知檢查結果有異常者，其尋求有照的中、西醫診所就醫率為80.2 %；未枕醫的最大理尚是自覺疾病不嚴重。
二、老人接受衛生所健檢稜，有慢性健康問題者，其中33.8％有多重就醫現象，主要係採用西醫治療。
三、影響受訪老人就醫的重要因素有（一）傾向因素：疚病易感性。口能力因素： 1.醫療保險，2.國定就醫場所，3.國定醫師就診，4.社會支持，5.醫師態度，6.介衛護士追蹤。口需要因素：1.健康問題總數，2.問題嚴重度，3.羅病期，4.疾病關心程度。

The purpose of this study was (1) to ascertain whether or not the elderly utilized the health care system and did follow-up after the initial free health check-up and screening; (2) to investigate the types of chronic health problems the elderly who utilize health care had and their reasons for using the health care system; and (3) to explore related factors that influence the utilization of health care by the elderly.
In this study, 484 elderly individuals were interviewed by home visits. All names were from the 11 public health clinics in Kaohsiung City, and all subjects had been diagnosed with one of the following disorders: heart disease, diabetes mellitus, hypertension, liver and biliary disorder, urinary disorder, or lung disease. Data
were statistically analyzed by means of descriptive and analytical methods, %, t-test, ChiSquare test and odds ratio.
The major findings of this study were as follows:
(1) the utilization rate by chronically ill elderly was 80.2%.
(2) elderly with multiple utilization of the health care system comprised 33.8%.
(3) the variables influencing the utilization of the health care system inclued: perceived susceptibility, fixed physician, medical insurance, doctor’s attitude, social support, community health nurse follow-up, the number of diseases, perceived severity of the illness, and attitude of health.
