精神分裂病之社會病程:區域與社會影響因素

摘要


作者等提倡一精神分裂病社會病程模式,主要係依據不同跟國文化研究,針對家庭、社會支持系統,工作、政治經濟狀況以及法律與醫療院所等對精神病經驗研究結果所成。作者等推論日常生活會影響精神分裂病之社會病程,一方面是因為個人痛苦表徵會受到文化因素的影響,另一方面個人疾病經驗之表達與溝通受社會因素的影響。作者等同時相信該模式可以推及至其他慢性疾病如憂鬱症、糖尿病、氣喘病、關節炎、慢性疼痛症候群、慢性疲勞症候群、心臟病及癌病等。上述人類學之方法可應用於研究臨床。

In this paper, we propose a model of social course of schizophrenia based on crosscultural research on the influence of family, wider social network, work, political economy, and legal and mental health care institutions on the experience of illness. We posit the way these ordinary arrangements of daily living organize the course of schizophrenia in part through cultural processes that affect the body-self in suffering and in part through social processes that establish an intersubjective matrix for the experience of illness. We believe this model can be generalized to other chronic illness such as depression, diabetes, asthma, osteoarthritis, chronic pain syndrome, chronic fatigue syndrome, and even heart disease and cancer. We develop the implications of this anthropological approach for research and practice.
