摘要

病人安全議題近幾年受到世界先進國家的重視。美國於1999年出版的”To Err is Human”提到美國每年死於醫療疏失的人數，遠高於因交通意外事故死亡的人數。故而「促進病人安全」漸漸被視爲醫療機構重要的醫療改革重點。以衛生署訂定的病人安全目標，發現正確給藥及高警訊藥物管理是促進病人安全的重點項目之一。在此前提下本院護理部環境安全組著手制定病患安全手冊，其中用藥安全的內容包括給藥及藥品安全、常備藥品及管制藥品管理、高危險藥品的認識及管理，供臨床護理人員工作參考。用藥錯誤爲醫療失誤中最常發生的事件，護理人員從事臨床工作應具有藥品辨識及警覺能力，全體醫療人員更應養成重視病忽安全的良好組織文化，以避免醫療錯誤發生。

Recently, the safety issue of patients has been a topic of focus in developed countries. A study published in 1999 ”To Err is Human,” mentions that in the USA, the number of people who died from medical malpractice is far more than that from traffic accidents. Therefore, the ”promotion of patients' safety” has become an important topic in the medical system. Improving the safety of medications and improving the safety of using high-alert medications are two patients' safety targets of the Taiwan's Department of Health. Under this premise, the patients' safety unit of nursing department publishes patients' safety manuals as guides for practicing nurses. The information covers medication safety, correct use of medications, management of general medications and controlled medications, recognition and control of high-risk medications. Wrong use of medication is the most frequent form of medical malpractice. Nurses should be competent in medication identification and be alert when performing clinical works. All health care professionals should value each patient's safety in order to avoid malpractice.
