鼻咽癌患者接受空腸造口術術後照護經驗
Nursing Experience of a Patient with Nasopharyngeal Carcinoma after Jejuostomy Operation
摘要
本文敘述一位鼻咽癌老年患者因曾接受放射治療導致吞嚥困難，無法由口進食而接受空腸造口手術後的護理經驗，筆者藉由觀察、評估及會談的方式，並運用Gordon 11項功能性健康型態評估後，擬定呼吸道清除功能失效、營養狀況改變、活動無耐力及無望感等護理問題。照護期間從91年2月28日~5月5日；藉由提供合宜的護理照顧，且積極與家屬合作，使之成爲照顧支持者之一，此外，在設計臥床老年患者的復健活動上，需以患者需要爲考量，運用生活用品的小創意，如筆者爲患者設計以寶特瓶及尼龍繩執行床上復健活動，使得患者手部活動力明顯增加，因而協助患者逐漸恢復生理、心理的安適，茲提出此護理經驗與護理同仁們分享。

This article is to present the nursing experience of an old patient after his jejunostomy operation because he encountered swallowing difficulties due to radiotherapy thus he could not be feed through mouth. Via observation, discussion with patient and evaluation using Gordon 11 functional health assessment guide, the author set down the nursing plan to help battle the malfunction of airway clearing, nutrition alteration, intolerance of activity persistence and hopelessness. The nursing period ranged from February 28 to May 5,2002. The entire procedure was to provide a proper nursing care plan and coordination with the patient's family, thus they play the big supporters of the entire caring procedure. Besides, to design the recovery activities for an old bed-ridden patient we needed to consider the individual's needs using some daily life articles with invention. For instance, the author once designed an on-bed recovery activity by using PET bottle and nylon thread to help improve the patient's hand mobility, thus he recovered physically and mentally in some degree. Through this article the author would like to share the nursing experience with all my colleagues.
