一位急性心肌梗塞患者行經皮冠狀動脈成型術之護理經驗
An AMI Patient after Undergoing PTCA: A Nurse's Experience
摘要
本文描述一位75歲急性心肌梗塞(Acute myocardial infarction, AMI)病患，接受緊急經皮冠狀動脈成型術(Percutaneous transluminal coronary angioplasty, PTCA)後住加護病房治療的過程。在照顧期間利用觀察、傾聽、會談、查閱病歷收集病史，運用Gorden十一項健康功能型態評估個案健康問題，發現病患有組織灌流改變、知識缺失、焦慮的健康問題。護理過程採集中護理、密切觀察生命徵象及病情進展以降低心臟負荷，心理支持、細心的PTCA術後照顧降低焦慮並提升病患及家屬對疾病的認識及自我照顧能力，藉此個案護理經驗，提供同仁照顧類似個案的參考。

This article describes a nurse's experience in an AMI patient, admitted to the intensive care unit after receiving percutaneous transluminal coronary angioplasty (PTCA) treatment. Information was collected through observation, listening, interviews and inspection of medical records. The patient's health problems were assessed according to the Gorden's model. The patient had various health problems: ineffective tissue perfusion, knowledge deficit and anxiety. I used the nursing knowledge and skills to observe the disease's processes closely and also provided post-PTCA care and appropriate education. I would like to share my experience with my coworkers when taking care of similar cases.
