一位末期腎病病人之護理經驗

本文探討一位長期服用中藥養身疑似馬兜鈴酸致末期腎病而需長期接受透析治療之病人，因對於疾病病因與治療不瞭解引發抉擇衝突之因應與調適過程，於2008年9月25日至10月30日照護期間，以Gordon's十一項健康功能型態為評估架構，藉由觀察、身體評估與會談等方式收集資料，歸納個案有特定與疾病相關的部分知識缺失、潛在危險性感染、抉擇衝突等健康問題。護理過程中與個案及家屬建立專業性人際關係，應用腎臟科護理專業知識，協助個案了解自己的疾病，從奮力抗拒到接受並選擇腹膜透析治療，成功地學會居家腹膜透析及自我照顧技能，並於出院後電訪關懷追蹤執行情形，展現慢性病人持續性照護之重要性。故而藉由此篇護理經驗提供護理同仁對長期服用中藥養身而引發末期腎病的瞭解，作為日後照護此類病人之參考。

The report covers the case of a patient who received peritoneal dialysis (PD) for ESRD after taking herbal medication (suspicious Fructus Aristolochia) for nutritional purposes for a long time. He was admitted from Sep. 25 to Oct. 30, 2008. We used ”Gordon's 11 items health functional status” as an evaluation frame, through observation, physical examination, and conversation to collect data. We found that the patient lacked knowledge of ESRD, specifically in regards to the risk of infection and potential decision conflicts. We built up an interpersonal relationship with the patient and the patient's family. Through nursing professional knowledge, we assisted the patient to understand ESRD and to accept PD as the treatment of choice. He successfully learned how to manage PD and self-care at home. Patient follow up is conducted via telephone interview. The study reveals the importance of persistent nursing care for patients with chronic diseases. This article details the nursing experience of patient care of ESRD caused by herbal medication.
