運用治療性遊戲於多發性齲齒幼童之手術全期護理經驗

Perioperative Nursing Experience of Using Therapeutic Play on a Child with Multiple Dental Caries Surgery

摘要


本個案報告為照護一位接受多發性齲齒手術幼兒之手術全期護理經驗,照護期間由觀察、會談、與案童及家屬互動、手術後訪視及電話訪談等方式,以Gordon十一項健康功能型態對案童身體、心理和家庭互動方面評估,確定健康問題為下列六項:(一)害怕;(二)焦慮;(三)潛在危險性皮膚完整性受損;(四)潛在危險性傷害;(五)舒適情況改變;(六)不遵從。針對健康問題運用治療性遊戲以海綿寶寶圖晝、夢夢狗口腔王國大冒險和魔法親親的故事,配合布偶運用角色扮演和假想性遊戲的方法營造溫馨的治療環境,與案童建立信任感。提供粗直徑的蠟筆和圖晝紙讓案童以繪圖宣洩情緒減輕害怕,協助案童面對手術,並運用牙刷超人大戰蛀牙菌的故事,將鹹蛋超人貼紙貼於牙刷,提供生動活潑的護理指導,帶領主要照顧者共同參與,改善案童對牙齒保健不遵從的問題。經過上述護理活動,發現的確可以幫助案童順利渡過整個手術全期,並且有助後續長遠的牙齒照護。

This case report concerns the perioperative nursing experience of a child who has undergone Multiple Dental Caries surgery. According to Gordon's 11 items of functional health patterns, we identified six health problems, including: (1) Fear, (2) Anxiety, (3) Potential risks to skin, (4) Potential risk of injury, (5) Changes of comfortable conditions, (6) Noncompliance or disobedience. We targeted these health issues and attempted to create a warm therapeutic environment and a feeling of trust with the case child by using puppets to encourage role play and imagination games. We provided a set of wide-diameter wax crayons and drawing paper to help the case child vent his emotions, and we used stories to achieve a lively and active nursing care. Through these strategies, we found that the case child was enabled to smoothing endure the entire period of the operation, and that there were lasting benefits to long-term follow-up dental care.
