摘要

本專案目的在提升某加護病房護理師洗手遵從率，藉由乾淨的雙手提供清潔照護，以減少發生院內感染，進而提升病人安全。分析現況發現常洗手過敏不舒服、工作太忙、有戴手套、一起做完再洗手、認知不足等問題，故著手進行改善方案，舉辦教育訓練、廣設乾洗手劑、提供護手乳、公告洗手時機、張貼洗手宣導海報、無菌盤包上置放洗手標示牌、電腦桌布及螢幕保護程式設計提醒洗手圖。護理師洗手遵從率由改善前31%提升到80%，達到目標值80%。希望藉此改善經驗，提供其他加護病房及其他工作人員參考，以全面提升洗手遵從率。

This project aimed to improve handwashing compliance rate among nursing staff in an intensive care unit, through cleaning hands, providing cleaning care, decreasing nosocomial infection, and improving patients' safety. It was found after observations, that incompliance was a result of several factors such as skin irritation due to frequent handwashing, no time for washing, wearing gloves, washing hands after finishing all works, and lack of knowledge. Thereafter, interventions such as handwashing continuing education were held, sterile alcohol solution was placed at the bedside and hand cream was placed at the sink of nursing station. Scheduling with announced hand washing timing, putting up poster at ward to encourage handwashing, putting tag in usual aseptic tray, and designing and storing the remaining draft in computer were applied. After implementation, the handwashing compliance rate increased from 31% to 80%, and the goal-reached rate was 80%. We are honored to share this project with other professionals in this field.
