醫院照顧服務人員對洗手之認知、態度與行為之探討 ─以某區域醫院為例

摘要

研究目的：了解醫院照顧服務人員照顧病患時對洗手知識、態度與行為表現的相互關係，並針對認知與態度較差的部分提出具體改善作為，以提供病患一個「以病人安全為中心」，具有良好品質的優質醫療環境。
研究方法：採橫斷式研究設計，自擬結構性問卷為調查工具，以新北市某區域醫院全院有照顧病患的照顧服務人員為研究對象，有效樣本數為213人。
研究結果：一、照顧服務員的臨床照護年資、學歷及服務科別的差異性，對於洗手認知、行為會有所影響；二、同儕間的洗手習慣、服務單位明確清楚的規範，對於洗手認知及態度有顯著之影響，三、主管的看法、病患的看法、及同儕間的互動支持、病患院內感染的情況變化回饋報告，皆是影響因子。
研究建議：一、將洗手教育訓練課程納入「年度照顧服務人員繼續教育」之必修課程中，並依人員之照護科別，做政策的促進與追蹤；二、培訓品管小組人員參與醫護人員洗手遵從性之監測；三、適時地回饋及獎勵；四、授權病人對本身照顧的責任；五、裝設電子監測系統，更能準確的測量工作人員洗手遵從率。

Objectives: To provide a “Patients Safety Oriented” high quality medical environment, this research targets nurse assistants, for investigating the relations among their percepion, attitude and behavior regarding hand hygiene, such that improvement approach could be proposed.
Methods: This study uses cross-sectional model to investigate. Research tool includes the self-made structural questionnaires, and the nurse assistants work at a regional hospital in New Taipei City as subjects. The number of effecitive samples is 213.
Results: 1. The difference of seniority, education degree, and working unit of the nurse assistants affect their perception and behavior regarding hand hygiene; 2. Hand washing habits among the peer, clear and definite working standard affect their perception and attitude regarding hand hygiene; 3. The points of views of the hospital managers and patients, the interactives and supports among the peers, feedback report of nosocomial infection situation all are the impact factors.
Suggestions: 1.Including “hand hygiene education and training” as mandatory course of annual nurse assistant personnel continuing education, and follow-up by working units of nurse assistants. 2.Training QC personnel participates in supervising the hand hygiene behavior obeying of hospital staff. 3.Proper feedback and reward.4.Authorize patients responsible for taking care themselves 5.Install electrical monitoring system to measure the obeying rate of hand washing of the personnel.
