應用跨專業領域團隊之實證醫療促進臨床病人安全及照護品質

跨專業領域團隊照護之實證醫療乃當前醫療環境中重要的照護趨勢，因來自單一專業領域的醫療並不足以提供以病人為中心之照護，更遑論健全全人照護。研究報告顯示，以跨領域團隊之實證照護為基礎，能讓各專業醫療人員瞭解相互尊重之團隊合作照護精神，透過彼此溝通，取得最佳治療策略，進而提供良好的照護品質。本文藉由探討跨專業領域團隊照護之緣由及定義，整合相關文獻具體闡述發展跨領域團體之實證實務的策略，提供臨床實務者及醫療機構領導者參考。本文期望藉此拋磚引玉，讓臨床護理人員及其他醫事團隊更熟悉跨領域團隊之實證醫療照護，充分具體應用於臨床實務上。

A transdisciplinary care model of evidence-based practice (EBP) has currently become the most important clinical care model. Single disciplinary approach could not provide a ”patient-centered care” as well as the holistic comprehensive care. Based on a transdisciplinary care model of evidence-based practice, different professionals can realize the value of collaboration and make the best clinical decision through efficient communication, and achieve better quality care for patients. In this article, we discussed the reasons and definition regarding transdisciplinary care model of evidence-based practice and integrated relevant literatures of strategies for development of transdisciplinary practices. We anticipate providing references for clinical practitioners and policy makers of clinics, helping nursing professionals and other disciplines to recognize ways to engage in transdisciplinary care of evidence-based practice and to apply it in clinical practices.
