Stroke can cause sequelae such as disturbance of consciousness, physical disability, and dysphagia. Due to the limitation of the patient's own abilities, this affects their daily life and causes their social ability to decline. In addition to physical problems, it will also cause considerable impact to their mind and family. This type of case amongst the elderly is very common in current Taiwanese society, and when the disease brings further physical and swallowing dysfunction, the basic needs of the patient cannot be satisfied, leading to negative emotions. The nursing period was from October 23 to November 20, 2018, through a cross-team model providing individual care, using teamwork to meet the patient's physical and mental needs, and assisting them to positively face the hardship of rehabilitation. The model successfully achieved the treatment goal by removing the nasogastric tube to meet the most desired basic needs of the patient, thereby improving the recovery of body function and life quality.
