提升腹部手術病人下床活動管路照護之完整性

摘要

醫策會台灣病人安全通報系統於2008年始將「提升管路安全」列為年度目標之一（醫院評鑑暨醫療品質策進會，2008，3月13日）。腹部手術病人下床活動時，臨床常見管路扭折、引流瓶傾倒情形，嚴重影響管路安全。本專案目的為提升腹部手術病人下床活動管路照護完整性。經由現況分析發現問題為護理人員管路照護認知不足與護理指導欠完整。故本案藉修訂腹部手術護理指導手冊、舉辦護理人員管路照護在職教育、製作管路照護指導單張及設計輪椅引流瓶固定帶以提升管路照護品質。實施結果顯示護理人員管路照護認知由79.5%增加至96.3%，腹部手術病人下床活動管路照護完整性由64.7%提升至87.5%，使得腹部手術病人下床活動時更加安心與安全。

The Patient Safety Committee of the Taiwan Joint Commission on Hospital Accreditation listed ”Promote Drainage Tube Safety” as the main target from the year 2008. Twisted tube and tilted drainage bottle were often seen in post-abdominal surgery patients during walking, which can seriously affect tube safety. This project tried to promote the integrity of tube care. Through data analysis we found the main problem to include: incomplete nursing instruction manuals, lack of knowledge in tube care among nurses, patients, and caregivers, ant the lack of equipment for drainage bottle placement on wheelchairs. Hence, we engaged in revision of the nursing instruction manuals, conducted service education for nurses, made drainage tube care handouts and designed the fixing bandage on wheelchair to promote tube safety. Result: the integrity of tube care improved from 64.7% to 87.5%. This project has been promoted to other units with positive results.
