摘要

本文是描述一位傷口遭海洋弧菌感染致壞死性筋膜炎個案之照護經驗，筆者以羅氏適應模式評估，發現個案有現存性感染、疼痛及無望感等問題。照護初期優先處理傷口感染及疼痛問題，除提供個案傷口照護、預防感染措施及疼痛緩解技巧外，並以同理心傾聽其表達內心感受，協助了解目前身體狀況以增加控制感，終使個案感染得以控制促進傷口癒合，並讓個案的疼痛減輕渡過無望感的危機，期望透過此護理經驗提供同仁日後照護參考。

This article describes the nursing experience of a patient wounded and infected with vibrio vulnifi cus, which led to necrotizing fasciitis. By using Roy's adaptation model, the major problems were identifi ed as existing infection, pain, and hopelessness. Early care prioritizes wound infection and pain; in addition to wound care for the patient, infection prevention measures and guidance for pain relief techniques were provided. Furthermore, we listened to the patient with empathy and helped the patient to understand the current body conditions to increase the patient's sense of control. Eventually, the infection was controlled, wound healing was improved, and the patient's pain was alleviated, enabling the patient to overcome the feeling of hopelessness.
