北部某醫學中心職業安全衛生室2014-2015年針扎統計,手術室扎傷佔全院30%,專案小組經現況分析發現醫護人員預防扎傷安全行為執行率僅達59%,原因如下:針頭不回套、穿戴雙層手套、傳遞尖銳物時先告知對方及使用持針器裝卸刀片/夾持縫針等預防扎傷安全行為執行率低;缺乏處理針頭設備及安全針具;未定期舉辦預防扎傷教育訓練及推動作業規範且無稽核機制。透過文獻查證及小組成員共同討論,以決策矩陣分析擬定多元教學策略,包含設計口號、有聲海報、拍攝及播放宣導短片、增加安全設備、規劃教育訓練及舉辦闖關活動並定期稽核預防扎傷安全行為等。專案執行後評值安全行為執行率由59%提升至91%成效良好。結合多元教學策略確實有效改善人員安全行為,讓最難改變的行為出現曙光,建議將此活動列入常規教育訓練以促進醫護人員工作安全。
The Occupational Safety and Health office of the hospital reported that from 2014 to 2015, sharp injuries in the operating room accounted for 30% of hospital injuries. Analysis revealed that the implementation rate of safety behavior to prevent sharp injuries by medical personnel was only 59%. The reasons were as follow: a low implementation rate of safe behavior to prevent sharp injuries, no needle-processing equipment and no safety-needle equipment, no regularly organized prevention of sharp injury education training, and no auditing mechanism. Through document verification and brainstorming of team members, diversified teaching strategies were developed on the basis of decision-matrix analysis, including adding security devices, designing slogans and audio posters, shooting and broadcasting video, planning education training, and organizing activities and regular audits. After project establishment, safety behavior implementation rate improved from 59% to 91%. Diversified teaching strategies improved staff safe behavior. These strategies can promote safety behavior among medical personnel.
