照護一位新住民面對家暴及多重創傷之護理經驗
Experience of Providing Care to a New Immigrant Facing Domestic Violence with Multiple Traumas

本文描述一位21歲新住民礙於語言障礙及文化差異，遭受家庭暴力，產生負面想法及無力感的感受，病人自住家2樓跳下，造成身體多處創傷骨折接受手術後的護理經驗，照護期間為2019年7月30日至8月15日，以Gordon十一項功能健康型態評估，採用觀察、身體評估、傾聽及病歷查閱等方式收集資料，確立主要護理問題為急性疼痛、身體活動功能障礙及個人因應能力失調。照護過程介入跨團隊合作共同擬訂照護計畫；給予藥物及非藥物治療降低疼痛；復健師介入積極復健，改善身體活動功能障礙問題；透過轉介社工尋得家屬支持，協助心理調適及處理社會問題，修正病人負面想法，恢復自信，順利出院。建議臨床照顧新住民併多重創傷病人可運用跨團隊合作模式，由多位專業人員以病人為中心共同評估，提供融合雙方文化差異之適切照護。

This article describes the nursing experience of caring for a 21-year-old new immigrant who suffered domestic violence due to language disability and cultural differences, resulting in negative feelings and feelings of powerlessness. She jumped off her building from the second floor of her house, which caused multiple traumas and fractures. Nursing care was provided from July 30 to August 15, 2019. Gordon 11 functional health assessments were used to collect data through observation, physical assessment, listening, and medical history review. The results revealed the following health problems: acute pain, impaired physical function, and ineffective individual coping. The nursing process involved cross-team cooperation to jointly formulate a care plan; providing medication and nonpharmacological therapy to reduce pain; interventions by rehabilitation therapists for active rehabilitation to improve physical dysfunction; seeking support from family members through referral to social workers; and assistance for improving mental health and coping with social problems. These measures would help the patient correct negative thoughts, increase self-confidence, and enable smooth discharge from the hospital. Clinical care for new immigrants with multiple traumas should be provided using a cross-team cooperation model implemented by a number of professionals. The team should collaboratively evaluate the patient and provide appropriate care that integrates cultural differences.
