COVID-19病人心理健康縱貫性追蹤研究-以中部某醫學中心為例

Longitudinal Tracking Study on the Mental Health of Patients With COVID-19 in a Medical Center in Central Taiwan

摘要


由於COVID-19病毒變異急速,使確診的住院病人隨著病情嚴重度變化,在隔離病室中所誘發的心理健康狀況,及出院後回歸染疫前的日常生活所面臨之心理壓力,需要持續關懷及追蹤。本研究旨在探討中部某醫學中心COVID-19病人住院期間及出院後的心理健康狀況變化程度。採回溯性研究設計,從臨床資料庫取得2021年5月10日至2021年7月10日COVID-19確診住院隔離新病人共34位的次級病歷資料,進行縱貫性追蹤分析依病情嚴重度分為輕症及重症兩組,分析自新入院至出院後一個月,每週評估一次的心理健康分數,追蹤兩組病人心理健康狀況的變化及差異。結果顯示自新入院至出院後心理健康分數,重症組在睡眠困難(p=.012)、焦慮(p=.016)、易怒(p=.048)、憂鬱(p=.017)、自卑(p=.024)的困擾程度較高,且住院後第四週的睡眠困難(p=.031),焦慮(p=.007),易怒(p=.032),憂鬱(p=.017),自卑(p=.023)最為明顯,出院後的心理健康分數呈現下降趨勢。護理人員在病人新入院隔離病房時,即進行心理健康評估,提供心理照護,持續追蹤至出院後一個月,病人的心理健康可維持穩定的狀態。

Because of the rapid mutation of the COVID-19 virus, the pandemic has persisted. As such, the mental health of patients with severe symptoms in isolation wards and of patients discharged from isolation wards warrants investigation. Such patients require continual psychological care and follow-up. This retrospective study investigated changes in the mental health status of patients with COVID-19 in a medical center in central Taiwan during hospitalization and after discharge. The medical records of 34 patients diagnosed with COVID-19 in hospital and quarantined from May 10, 2021, to July 10, 2021, were obtained from a clinical database. In the two groups of severe cases, mental health scores had been assessed weekly from admission to 1 month following discharge. Changes in the mental health status of the two groups were analyzed. The results comparing severe group mental health scores at admission to those following discharge indicated the following results: sleep difficulties (p= .012), anxiety (p= .016), irritability (p= .048), depression (p= .017), low selfesteem (p= .024). Four weeks following discharge, the following results were observed: sleep difficulties (p= .031), anxiety (p= .007), irritability (p= .032), depression (p= .017), and inferiority complex (p= .023). Mental health scores decreased following discharge. Patients newly admitted to the isolation ward should undergo mental health assessments from nursing staff and should receive adequate psychological care. Stable patient mental health can be maintained through such care.
