本研究探討骨癌青少年與年輕成人接受肢體保留術後於身體活動量與生活品質之變化及影響因素。採縱貫性研究設計,研究對象為北部某醫學中心兒童血液腫瘤科病房,接受第一次肢體保留手術之原發性骨癌青少年與年輕成人,分別於其手術前、手術後第一個月及第三個月以國際身體活動量表台灣短版和SF-36健康生活品質量表,進行資料收集與分析。研究結果發現,身體活動量在三個時間點上皆未達統計上顯著差異(p=.517);生活品質之生理健康面向三個時間點達統計上顯著差異(p<.001),經多變項分析結果顯示術後一個月較術前有統計上顯著差異(p<.001);術後第三個月較手術前無統計上顯著差異,但隨著時間進展,生活品質之生理健康面向變化是先下降再提升。身體活動量與生理健康面向方面是無顯著差異(p=.070),顯示身體活動量愈多,對生活品質之生理面向是沒有影響;在身體活動量與生活品質之心理健康面向是有顯著差異(p<.001)。研究結果可做為臨床護理人員提供骨癌患者肢體保留術病人術前及術後照護之參考。
This study assessed the physical activity and quality of life of adolescents and young adults with bone tumors after they underwent limb salvage surgery. A longitudinal research design was adopted, and the participants were adolescents and young adults with primary bone tumors who underwent limb salvage surgery for the first time who were recruited from the children's hematology and oncology ward of a medical center in northern Taiwan. Data were collected using structured questionnaires at three time points, namely 1 month before surgery, 1 month after surgery, and 3 months after surgery. The International Physical Activity Questionnaire (Taiwan Version) and the Short Form (36) Healthy Survey were used as research tools. The study results revealed no significant difference in the total amount of physical activity at different time points (p= .517). However, overall physical health-related quality of life significantly differed (p< .001) at the three time points. Overall physical health was observed (p< .001) at 1 month after the surgery relative to that before the surgery (p< .001). No significant difference was observed at 3 months after the surgery relative to that before the surgery; however, overall physical health gradually improved over time. Furthermore, no significant association was noted between physical activity equivalent and overall physical health (p= .070), indicating that higher levels of physical activity has no effect on overall physical health. However, the association between physical activity metabolic equivalent and overall mental health was significant (p< .001), indicating that higher levels of physical activity were associated with poorer overall mental health. The study results may serve as a reference for clinical nurses for providing preoperative and postoperative care for patients undergoing limb salvage surgery.
