提升新進護理師學習Pig-tail引流照護之改善方案

Project for Improving the Quality of Learning Pigtail Drainage Care Administered by New Nurses

摘要


Pig-tail為胸腔科常見重要管路,剛進入職場的新進護理師在接受課程與技能指導後,實際照護病人時技能正確率僅55.7%,因而引發改善動機。經現況分析原因為對於Pig-tail引流照護知識與臨床情境無法連結、缺乏Pig-tail管路固定及連接練習教具、管路固定及連接方式缺乏圖示可參考、引流管連接方式不易記憶。經由舉辦Pig-tail引流照護工作坊、製作Pig-tail引流之人型模組提供練習、製作Pigtail管路連接與管路固定步驟流程圖卡、發想創意口訣方便記憶引流管連接方式,以提升新進護理師Pig-tail引流照護技能正確率。結果發現,新進護理師Pig-tail引流照護認知與技能正確率提升至100%,達到本專案設定之目標值,因執行成效良好,建議可定期舉辦工作坊及持續採用以上策略,以維持長期之學習成效及照護品質。

Pigtail drains are commonly employed in thoracic medicine; however, they can be difficult to administer, especially among inexperienced nurses. After new nurses who had just entered the workplace and received courses and technical guidance, their technical accuracy in caring for patients was only 55.7%, which triggered the motivation to improve. We investigated the factors influencing the effective administration of pigtail drains. The observed factors were (1) unable to connect Pig-tail drainage care knowledge with clinical scenarios, (2) lack of accessible learning tools related to the administration of pigtail drains, (3) lack of diagrams regarding how to attach pigtail drains, and (4) lack of ease in remembering fixation methods. Through our pigtail drainage education courses-which involved the use of mannequins for practice, the development of creative mnemonics, and the creation of diagrams to demonstrate the steps of attachment and fixation for pigtail drains-the aforementioned correctness rate increased to 100%. Accordingly, this paper suggests that regular on-the-job education and training can facilitate improvements in learning effectiveness and care ability in new nurses.
