檢體異常重送會造成檢驗值延遲發送及影響診斷處置決策,嚴重危害病人安全。本單位2022年1-6月血液檢體重送率高達1.29%,故引發改善動機,期盼運用多元化策略,改善血液檢體重送率。經現況分析發現原因為認知不足、檢體試管未直立放置、真空採血器使用意願低、無固定抽血用物擺放位置及缺少稽核制度。於2022年8月1日至2023年7月31日實施改善對策:透過製作教學影片及導入「目視化設計概念」製作攜帶式直立檢體放置盒、舉辦教育訓練、設置真空採血器放置位置、製作提醒單張、運用檢體採集車及實施稽核制度。專案實施後血液檢體重送率由1.29%降為0.52%,達專案目標並提升病人安全。
Resending abnormal blood specimens can delay the delivery of test results, potentially impacting diagnosis and posing risks to patient safety. Between January and June 2022, the resending rate of blood specimens in the intensive care unit reached 1.29%. This study aimed to implement strategies to reduce the resending rate of blood specimens. The study results indicated several key factors contributing to the high resending rate, including inadequate awareness, improper placement of specimen tubes, reluctance to use vacuum blood collection devices, lack of designated positions for blood drawing supplies, and the absence of an auditing system. To address these problems, improvement strategies were implemented from August 1, 2022, to July 31, 2023. These strategies included producing instructional videos, introducing "visual design concepts" to create portable vertical specimen placement boxes, conducting educational training, establishing designated positions for vacuum blood collection tubes, creating reminder leaflets, utilizing specimen collection vehicles, and instituting an auditing system. As a result of these initiatives, the resending rate of blood specimens decreased considerably from 1.29% to 0.52%.
