Effect of Ouabain with and without Perphenazine in Acute Cardiac Failure of heart-lung Preparations of the Dog

哇巴因(Ouabain)與哇巴因及2-氯-10｛3-［4-（β-羥乙基）六氫吡嗪］丙基｝苯噻嗪(Perphenazine)合用對犬心肺標本心肌衰竭作用之比較

摘要

用犬之心肺標本，先以戍巴比妥鈉，使心肌衰竭，再單用哇巴因，或合用哇巴因與PERPHENAZINE處理，以測定PERPHENAZINE對哇巴因之療效，有無影響。比較標準如下：(1)恢復主動脈流量（不包括冠狀血管流量）及心力充沛指數(Competence index)，所需哇巴因之平均劑量(Ⅰ)；(2)恢復心臟功能所需之平均時間（分）(Ⅱ)；(3)心力充沛指數，及主動脈流量恢復之持續情形；(4)心律不齊之有無。結果為：哇巴因，PERPHENAZINE合用，略增標準Ⅰ及Ⅱ所列者，但無統計上之意義。而心臟功能恢復後持續情形，則在哇巴因，PERPHENAZINE合用時較佳。且在哇巴因，PERPHENAZINE合併處理之標本，皆無心室纖維性亂縮現象。單用哇巴因處理實驗中，有兩實驗，發生心律不齊，經用PERPHENAZINE處理，即恢復正常，另有三實驗，也呈心律不齊，但不用PREPHENAZINE，則心律不齊，皆持續存在，而最後呈心室纖維性亂縮。

The efficacy of ouabain and ouabain with perphenazine in restoring a failing heart induced with sodium pentobarbital to its maximum functional level was compared in dog's heart-lung preparations with the following criteria: (a)average doses of ouabain required to restore the systemic output(minus coronary flow)and the competence index to their maximal level(Ⅰ), (b)average time required for the maximal effects to appear(Ⅱ), (c)the maintenance of the competence index and systemic out-put, and. (d)the presence or absence of any aberrant rhythm. It was found that while (Ⅰ) and (Ⅱ) were slightly increased with perphenazine given concomitantly with ouabain, there was no statistical difference bteween the ouabain- and ouabain-perphenazine-treated groups. The restored competence index and systemic output were, however, maintained better in the ouabain-perphenazine group than in the ouabain group. Furthermore, no ventricular fibrillation was observed in the ouabain-perphenazine group. In two experiments cardiac irregularity following ouabain was promptly reverted to regular rhythm with perphenazine. In three experiments the untreated cardiac arrhythmias following ouabain persisted, ending in ventricular fibrillation.
