影響臺灣老人存活相關因子探討－脆弱韋伯時間相依模型之應用

摘要

本研究使用臺灣衛生署國民健康局之「臺灣地區老人保健與生活問題調查」前後共十四年的五期追蹤調查資料（1989、1993、1996、1999及2003）進行影響老人存活相關因子的探討。利用足以代表臺灣地區60歲以上高齡者，歷經十四年觀察其存活及死亡狀況的4,049筆個案資料，以Cox模型進行分析，經模式選擇，選出年齡、性別、配偶、族群、抽菸、ADL（activities of daily living，日常生活能力）狀況、體能狀況、健康自評狀況等與老人存活有密切相關的8個變數。又，Cox模式中經共變數調整後之基準存活函數ln(-ln So(t))與ln(t)圖形大致爲直線，顯示此一資料符合韋伯參數模型。此外，本研究係屬時貫性調查資料有些變數會因時間而改變，因而引入時間相依變數於模式中，另基於未知共變數之個體異質性，本研究中考慮加入不同調查期中個案間未被觀察到的脆弱隨機效果因子，利用脆弱韋伯時間相依模型分析影響臺灣地區老人存活之相關因子，深入討論五次調查中受訪者之背景特徵、身心狀況、家庭環境、社會參與及衛生行爲等重要存活相關因子隨著時間變動影響老人存活風險。

This study aims to investigate the factors associated with the survival status of the elderly in Taiwan. We used the survey data, which is the Survey of Health and Living Status of the Elderly in Taiwan from 1989 to 2003, to explore the effects on the survival status of the elderly. Based on Cox proportional hazard model with time-dependent covariates, there are eight variables (age, gender, ethnic group, Activities of Daily Living (ADL), self-rated health, physical function, smoking and marital status) were found to be strongly related to the survival status of the elderly. Moreover, the linear relationship between ln-ln S0 (t) and ln (t) for each covariate in the Cox regression model was observed, which indicated that the Weibull distribution was fitted well for the data. Taking into account of the latent effect due to individual difference, the Weibull frailty model with time-dependent covariates was used to explore the effect prognostic factors such as demographic characteristics, health status, health behavior, home condition, and social participation, on the survival status of the elderly in Taiwan.
