Hemangiomas of the pectoralis major muscle are rare. We present a 23-year-old man who had an enlarged and lobulated lesion in left-sided breast and led to a cosmetic problem. The ultrasound of the breast and computed tomography of the chest showed a soft tissue tumor with multiple spotted calcifications of left-sided anterior chest wall. The correct diagnosis of cavernous hemangioma of the pectoralis major muscle was established by incisional biopsy and subsequent pathologic interpretation. Although intramuscular hemangiomas are rare, they should be included in the differential diagnosis of chest wall tumors. The treatment of choice for intramuscular hemangiomas is complete surgical excision.
