Intradural Lumbar Disc Herniation Mimicking an Intradural Spinal Tumor: Report of a Case
與脊椎硬脊膜內腫瘤相似之腰椎硬脊膜內椎間盤突出症：病例報告
摘要
硬脊膜內椎間盤突出症是椎間盤破裂很少見而且很嚴重的併發症。這種疾病通常發生在腰椎。雖然注射顯影劑之磁振造影是一個非常有用的診斷工具，但是在術前做出正確的診斷仍然相當困難，通常在手術中才能確定診斷。一位73歲女性下背痛及左腳疼痛、無力已有幾個星期的時間。注射顯影劑之腰椎磁振造影發現腰椎第二、三節間有一個硬脊膜內病灶，手術時發現是硬脊膜內椎間盤突出而且全部切除，病人術後恢復良好。在鑑別診斷脊椎內腫瘤時，硬脊膜內椎間槃突出必須加以考慮。

Intradural disc herniation is a rare, serious complication of intervertebral disc rupture that often occurs in the lumbar region. Although magnetic resonance imaging (MRI) with gadolinium is useful in the diagnosis of this condition, accurate preoperative diagnosis is still difficult, and the definitive diagnosis must be made during surgery. We describe a 73-year-old woman who had experienced severe low back pain with radiation to the left lower limb and mild weakness for several weeks. MRI with gadolinium revealed an intradural soft-tissue mass at L2-3. During surgery, intradural disc material was found and completely removed. The patient had an excellent outcome. Intradural disc herniation should be considered in the differential diagnosis of an intraspinal tumor.
