Epidural Abscess after Lumbar Discography: A Case Report

Discitis after discography has been reported in the literature as a well-known complication, yet epidural abscess is infrequent. However, early diagnosis and treatment of the infection should be performed as soon as possible. We report a 24-year-old male with lower back pain, initially treated with physiotherapy during an outpatient clinic follow-up. After failure of conservative treatment, discography was used for clinical evaluation of lumbar disc disease. The rare complication after a discography-epidural abscess - occurred and presented as severe back pain and high fever. Soon after, the diagnosis was confirmed by magnetic resonance imaging. After a laminectomy and antibiotic treatment, the infection improved immediately. The patient returned to work 1 month later uneventfully. The purpose of the paper is to present a case report on the uncommon, pyogenic spinal infection after discography. This complication also emphasizes the significance of prophylactic antibiotic therapy and the importance of aseptic surgical technique. Once an epidural abscess developed, surgical decompression and debridement should be performed.
