Vertebral compression fracture secondary to osteoporosis is the epidemic of this century in the western and Asian countries. The treatment of vertebral compression fractures has been changed during the past years. Vertebroplasty and kyphoplasty are now extensively used in Taiwan and universally. They are efficient treatment, but no free of complications. Intrathecal cement leakage is a rare, but devastating complication. We report a case of kyphoplasty with epidural and intradural leakage of bone cement complicated with myelopathy and radicular pain. The operation of laminectomy with removal of leaked cement was performed. The patient received regular out patient department follow-up and rehabilitation. And the symptoms were gradually resolved six months later. In our case, early surgical intervention could improve the result.
