Sleeve Pulmonary Artery Resection and Sleeve Lobectomy in the Treatment of Non-small Cell Lung Cancer: Report of Two Cases
袖式肺動脈切除及袖式肺葉切除治療非小細胞肺癌：兩病例報告
摘要
靠近中央區的非小細胞肺癌不只會侵犯支氣管而且會侵犯肺動脈。袖式肺葉切除己經被廣泛應用在保留遠端肺組織來避免全肺葉切除。但是同時進行袖式肺動脈及袖式肺葉切除來保存遠端肺組織的報告則不常見。我們報告兩例左上肺葉非小細胞肺癌患者同時接受袖式肺動脈及袖式肺葉切除。我們使用小的非創傷性血管鉗夾住肺動脈及肺問動脈。手術過程中及手術後均未使用抗凝劑。兩位患者術後恢復良好而且分別在28個月及6個月追蹤期間未見到復發。

Central-located non-small cell lung cancer may involve not only the bronchus but also the pulmonary artery. Sleeve lobectomy is a widely used surgical technique for preserving lung parenchyma. However, sleeve lobectomy combined with sleeve pulmonary artery resection to preserve the distal lung is not common. We report two patients who underwent sleeve lobectomy of the left upper lobe and sleeve resection of the pulmonary artery to treat non-small cell lung cancer of the upper lobe of the left lung. We used small atraumatic vascular clamps to control blood flow through the pulmonary and interlobar arteries. No heparin was used during or after operation. Both patients recovered well and were tumor-free 28-months and 6-months respectively after surgery.
